Option A – 15,000 NTD ( Limited to 80 Registrations ) :
The fee includes：
● live and pre-recorded operative demonstrations
● theoretical teaching and/or workshop
● coffee breaks and lunches
● one evening meal
● shuttle bus service provided
Option B – 70,000 NTD ( Limited to 40 Registrations ) :
The fee includes：
● live and pre-recorded operative demonstrations
● theoretical teaching and/or workshop
● practical training on live tissue (mini pigs)
● coffee breaks and lunches
● one evening meal
● shuttle bus service provided
*All payments need to be made in New Taiwan Dollars.

| Accommodation |
Accommodation is available at special AITS rates at the following choices (we will arrange another hotel when hotel fully booked):
- Windsor Hotel Taichung 3,400 NTD per night, single room, breakfast included
(Please make direct payment to Windsor Hotel).
- LOHAS Relaxing Center 2,000 NTD per night, single room, breakfast included
(Please make direct payment to AITS in advance).
※Accommodation is not included in course registration fee.
※Co-shared Pick-up Service is under individual request and personal expense. The fee is 750 NTD per person/ trip (only limited at Taichung and Taiwan Taoyuan International airport). Please provide with the flight schedule in advance and pay directly to AITS in CASH on site. Please do let us know if you would have the different arrival airport or destination.

| Registration |
On-line registration is recommended with payment on visa/master card. For paper registration, please send us your registration form duly completed
by fax (+886-4-707-3222) and/or postal mail.
Registration is confirmed upon receipt of the form duly filled out, signature affixed, with payment enclosed.
For organizing procedure, any cancellation of registration less than 30 days prior to the course starting date will NOT be transferred, reimbursed or postponed, except for the absence caused by the natural disaster.
